How do I get my Medical Marijuana Card?

1 BOOK AN APPOINTMENT

After reviewing the qualifying conditions, complete the form to schedule an appointment for your medical exam. You will need your medical records and valid identification for approval.

2 CONSULT WITH A DOCTOR

A 10-15 minute consultation with a physician or nurse practitioner will determine your eligibility for a medical marijuana card. The price of the evaluation is $200. Upon approval, the provider will complete a certification form for medical marijuana you will use to apply with the Mississippi State Department of Health. A six-month follow-up appointment with the provider is required.

3 GET APPROVED

Once you are approved by the provider, you will register with the state and submit an application. The state will determine your final approval and grant your medical marijuana card in the mail. An annual follow-up appointment with the provider is required to recertify.
