Qualifying Conditions

Do you have any of the following medical conditions?

Cancer | Parkinson's Disease | Huntington's Disease | Muscular Dystrophy | Glaucoma | Spastic Quadriplegia | HIV | AIDS | Hepatitis | ALS

Crohn's Disease | Ulcerative Colitis | Sickle-cell Anemia | Alzheimer's disease | Agitation of Dementia | PTSD | Autism

Cachexia or Wasting Syndrome | Chronic Pain | Severe or Intractable Nausea | Seizures | Severe and Persistent Muscle Spasms

Diabetic/Peripheral Neuropathy | Spinal Cord Disease or Severe Injury

Pain Refractory to appropriate Opioid Management

Age Restrictions

Anyone 21 and over who has any of the qualifying conditions can participate in the program. Any patient under the age of 21 must-have consent from a parent or guardian to participate.

Criminal History

Anyone who has been convicted of a violent crime may be ineligible. Previous illegal drug possession or distribution conviction at the federal or states level may cause ineligibility.
